Use this checklist to qualify subcontractors before awarding work. Proper qualification prevents problems.


Company Information

Basic Info

  • Company name - Legal business name
  • Business license - Current, valid license
  • Years in business - Experience level
  • Company size - Number of employees
  • References - 3-5 recent project references

Financial

  • Financial statements - Recent financials reviewed
  • Credit check - Credit report obtained
  • Bonding capacity - Bonding limits verified
  • Banking - Bank references checked

Insurance

  • General liability - Minimum $1M/$2M
  • Workers' compensation - Current, adequate coverage
  • Auto insurance - If using vehicles
  • Umbrella - Excess coverage (if required)
  • COI received - Certificate of insurance

Safety

  • EMR - Experience modification rate checked
  • OSHA record - OSHA violation history reviewed
  • Safety program - Written safety program
  • Training - Safety training records
  • Incident rate - TRIR (Total Recordable Incident Rate)

Work Quality

  • Portfolio reviewed - Past work samples
  • References checked - Called references
  • Site visits - Visited completed projects
  • Warranty - Warranty terms acceptable

Contract Terms

  • Scope clear - Work scope clearly defined
  • Price fair - Competitive pricing
  • Payment terms - Terms acceptable
  • Schedule - Can meet schedule
  • Warranty - Warranty terms acceptable
